Independent Healer & Provider Participation Agreement

- Purpose and parties
This agreement is between Holistic Healing 4 Life (HH4L) and the submitting healer, practitioner, or provider. It establishes participation in a collaborative community network and is not a franchise.
- Independent relationship
The provider remains independent and keeps control of identity, scheduling, pricing, clients, records, methods, and professional judgment. This agreement does not create employment, agency, a partnership entity, franchise, or joint venture.
- HH4L network support
Based on selected choices and available opportunities, HH4L may provide visibility, introductions, classes, demonstrations, events, educational content, collaboration, cross-referrals, and participation in human wellness, pet wellness, or both.
- Provider responsibilities
The provider is responsible for required licenses, credentials, insurance, permits, disclosures, consent, privacy, records, taxes, advertising, lawful scope, accurate information, respectful service, and reporting material changes.
- Participation and publication
No profile or offer is published until final listing details are supplied and confirmed. The provider authorizes HH4L to use confirmed business information and promotional materials and may request reasonable correction or removal.
- Member offer
Any member discount or special offer must be written here or later confirmed in writing. The provider controls eligibility and is responsible for honoring the confirmed offer. No discount is assumed.
- Optional growth programs
Customer-database services, SenText Capital funding, and CareCredit enrollment are separate and optional. Each may require separate enrollment, qualification, approval, fees, eligibility, and written terms.
- No guarantees and client choice
HH4L does not guarantee referrals, clients, income, employment, events, funding, CareCredit approval, or business results. Clients choose their provider. The provider remains responsible for services, outcomes, billing, refunds, disputes, and care.
- Term and ending participation
The agreement begins when signed by the provider and accepted by HH4L. Either party may end participation by written notice. HH4L may pause or remove a listing for safety, legal, credential, conduct, accuracy, or reputation concerns.
- Complete agreement and signatures
This online agreement and later written participation details form the agreement. Changes must be in writing. Electronic and typed signatures may be accepted as originals. A meeting is for onboarding and support, not a condition for deciding to join.
This practical business agreement is not a substitute for legal advice. HH4L and the provider may each have an attorney review it before signing.
